This waver covers offsite events like bowling, mini-golf, joint events with other youth groups around the St. John's area. 

This waiver is only required once per year.

Avalon Youth Retreat Registration & Waiver

This waver is required for anyone under 18 years of age attending AYR  with Lytehouse Youth Ministries. 

This waiver includes: 

  • Registration Form
  • Letter of Informed Consent 
  • Permission Form & Consent, and 
  • Off-Site Travel Form
